Consent forms

A consent/release form is a sheet of paper that requires personal and contact details of participants in a production. They are necessary because we need to have the actor's/model's permission in a production recorded on documents to make the process confidential. 

I made a simple consent form with Microsoft Word. For privacy reasons, I have concealed personal information such as their address, phone number and signature.
